CPR Roster Retention Requirements: AHA ACLS Course Roster form

The AHA requires a special 3 years preparation for the following course records:

• verified copies of completed course CPR rosters, you should keep a copy for instructor recertification purposes;
• exanimate skill performance sheets degree;
• written exam answer sheets after all admission criteria;
• course verification / evaluations;
• documentation related to problem of resolution;
• evidence of use of the AHA ACLS Course Roster form fee disclaimer for courses in which fees are charged;

AHA ACLS Course Roster form Fees:

The AHA ACLS Course Roster form does not set or receive fees for any courses. The following disclaimer should be printed on all course promotional brochures, posters, catalogues, announcements, agendas, or other materials distributed to students in courses for which fees are charged:

"The American Heart Association (AHA ACLS Course Roster form) strongly actually promotes knowledge and proficiency in BLS, ACLS, and PALS and has developed instructional materials for this purpose. Use of these materials in an educational course does not represent course sponsorship by the American heart Association. Any fees charged for such a course, except for a portion of fees needed for AHA ACLS Course Roster form material, do not represent income to the Association."
